5D Consent Form – Research

Intro

The Consent Form for the Action research Project “Growing Equity”, to hand out to the participants in an the action research project, is focused on the impact of student self-evaluation on critical thinking anxiety and compassion among diverse student groups. It outlines the purpose of the research, what participants will be asked to do, how data will be stored and used, participant quoting and referencing preferences, optional disability accommodations, consent to participate, including image or voice recording, and the right to withdraw. Participants are required to indicate their consent preferences and provide a signature, name or code for anonymity, and the date. The Consent Form adheres to the UAL’s Code of Practice on Educational Ethics outlined in the Ethical Enquiry Form provided in my Ethic‘s blog post.
